  2. Identification of the copyrighted work claimed to be infringed
  3. Identification of the allegedly infringing material with enough detail to locate it on the site
  4. Your contact information (name, address, telephone, email)
  5. A statement that you have a good-faith belief the use is not authorized
  6. A statement under penalty of perjury that the information is accurate and you are authorized to act

Counter-Notice
